A government appointed panel has accused the US State Department of not doing enough to protect its diplomats in Libya.
The panel investigated a series of attacks on the US mission in Benghazi in September, including one that killed the US ambassador to Libya and three members of his staff.
But firing someone was not on the panel's list of 29 recommendations.
